Medical and Liability Release Waiver
I, the parent/legal guardian, of the player, a minor, or a player age 18 or over, agree that I and the player will abide by the rules and regulations of U.S. Club Soccer and U.S. Youth Soccer (USYS), and their affiliated organizations, NorCal, and the California Youth Soccer Association, Inc (CYSA), and its affiliated organizations, and Conscious Athlete Academy (hereinafter “Conscious Athlete Academy”). I, for myself and minor player, and each of their respective heirs, administrators, assigns and successors, intending to be legally bound, hereby agree to release and indemnify and hold harmless Conscious Athlete Academy and its officers, board members, employees, agents and representatives, NorCal, US Club Soccer, USYS and CYSA Parties, the owners and operators of the fields used for the Programs, and their respective directors, officers, employees, agents, and representatives from and against any and all claims, liabilities, damages, or causes of action arising out of or in connection with the player’s participation in the Programs including, in those instances where Conscious Athlete Academy is negligent, and including without limitation, player’s transportation to/from any Program, which transportation is hereby authorized. As the parent/legal guardian of the above-named player, or player age 18 or over, I hereby give consent for emergency medical care prescribed by a duly licensed Doctor of Medicine or Doctor of Dentistry. This care may be given under whatever conditions are necessary to preserve the life, limb or well-being of me or my dependent. I further grant Conscious Athlete Academy, US Club Soccer, USYS and CYSA Parties the right to use player’s name, picture and /or likeness in printed, broadcast and other material concerning the Programs provided such use is related to the player’s status as a participant in the Programs.

Yo, el padre/tutor legal del jugador, un menor de edad, o un jugador mayor de 18 años, aceptó que tanto yo como el jugador acataremos las reglas y regulaciones de U.S. Club Soccer y U.S. Youth Soccer (USYS), así como de sus organizaciones afiliadas, NorCal y la Asociación de Fútbol Juvenil de California, Inc. (CYSA), y sus organizaciones afiliadas, y Conscious Athlete Academy (en adelante "Conscious Athlete Academy"). Yo, en mi nombre y en el del jugador menor de edad, y cada uno de sus respectivos herederos, administradores, asignados y sucesores, con la intención de quedar legalmente obligados, por la presente acuerdo en liberar, indemnizar y eximir de responsabilidad a Conscious Athlete Academy y a sus funcionarios, miembros de la junta directiva, empleados, agentes y representantes, NorCal, US Club Soccer, USYS y las partes de CYSA, los propietarios y operadores de los campos utilizados para los Programas, y sus respectivos directores, funcionarios, empleados, agentes y representantes de y contra cualquier y todos los reclamos, responsabilidades, daños o causas de acción derivados de o en conexión con la participación del jugador en los Programas, incluidos, en aquellos casos en que Conscious Athlete Academy sea negligente, e incluyendo, sin limitación, el transporte del jugador hacia/desde cualquier Programa, cuyo transporte queda autorizado por la presente. Release of Liability and Assumption of Risk Agreement Relating to Coronavirus/COVID-19
In consideration of being allowed to participate on behalf of Conscious Athlete Academy athletic program and related events and activities, the undersigned acknowledges, appreciates, and agrees that:
Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and,
I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,
I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe and any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,
I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Conscious Athlete Academy, their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
I also agree to follow all safety requirements that the soccer training or summer camp imposes as a condition of enrolling my child.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
FOR PARTICIPANTS OF MINORITY AGE (UNDER AGE 18 AT THE TIME OF REGISTRATION)
This is to certify that I, as parent/guardian, with legal responsibility for this participant, have read and explained the provisions in this waiver/release to my child/ward including the risks of presence and participation and his/her personal responsibilities for adhering to the rules and regulations for protection against communicable diseases. Furthermore, my child/ward understands and accepts these risks and responsibilities. I for myself, my spouse, and child/ward do consent and agree to his/her release provided above for all the Releasees and myself, my spouse, and child/ward do release and agree to indemnify and hold harmless the Releasees for any and all liabilities incident to my minor child’s/ward’s presence or participation in these activities as provided above, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent provided by law.

En consideración de ser permitido participar en nombre del programa atlético de Conscious Athlete Academy y eventos y actividades relacionadas, el abajo firmante reconoce, aprecia y acepta que:
La participación incluye la posible exposición e enfermedades infecciosas, incluyendo pero no limitado a MRSA, influenza y COVID-19. Aunque ciertas reglas y disciplina personal pueden reducir este riesgo, el riesgo de enfermedad grave y muerte sí existe; y,
ACEPTO CONOCER Y ASUMIR LIBREMENTE TODOS ESTOS RIESGOS, tanto conocidos como desconocidos, INCLUSO SI SURGEN DE LA NEGLIGENCIA DE LOS LIBERADOS o de otros, y asumo plena responsabilidad por mi participación; y,
Acepto voluntariamente cumplir con los términos y condiciones establecidos y habituales para la participación en lo que respecta a la protección contra enfermedades infecciosas. Sin embargo, si observo cualquier riesgo inusual o significativo durante mi presencia o participación, me retiraré de la participación y lo informaré de inmediato al oficial más cercano; y,
Yo, por mí mismo y en nombre de mis herederos, asignados, representantes personales y familiares más cercanos, POR LA PRESENTE LIBERO Y EXIMO DE CULPA a Conscious Athlete Academy, sus oficiales, funcionarios, agentes y/o empleados, otros participantes, agencias patrocinadoras, patrocinadores, anunciantes, y si corresponde, propietarios y arrendadores de las instalaciones utilizadas para llevar a cabo el evento ("LOS LIBERADOS"), CON RESPECTO A CUALQUIER Y TODA ENFERMEDAD, DISCAPACIDAD, MUERTE, o pérdida o daño a la persona o propiedad, YA SEA QUE SURJA DE LA NEGLIGENCIA DE LOS LIBERADOS O DE OTRO MODO, en la medida máxima permitida por la ley.
También acepto seguir todos los requisitos de seguridad que el entrenamiento de fútbol o campamento de verano imponga como condición para inscribir a mi hijo.
HE LEÍDO ESTE ACUERDO DE LIBERACIÓN DE RESPONSABILIDAD Y ASUNCIÓN DE RIESGOS, COMPRENDO PLENAMENTE SUS TÉRMINOS, ENTENDIENDO QUE HE RENUNCIADO A DERECHOS SUSTANCIALES AL FIRMARLO, Y LO FIRMO LIBRE Y VOLUNTARIAMENTE SIN NINGÚN INCENTIVO.

Certifico que yo, como padre/tutor legal, con responsabilidad legal sobre este participante, he leído y explicado las disposiciones de esta renuncia/liberación a mi hijo/dependiente, incluyendo los riesgos de presencia y participación y sus responsabilidades personales para adherirse a las reglas y regulaciones para protección contra enfermedades transmisibles. Además, mi hijo/dependiente comprende y acepta estos riesgos y responsabilidades. I grant Conscious Athlete Academy permission to take and use photographs, videography and/or digital images of my child for use in:
Media releases, media articles – including newspapers, radio, television – printed publications, signage and/or educational materials
Electronic publications and communications such as the club’s Instagram, TikTok, Facebook site and website
I agree that my child’s name and identity may be revealed in descriptive text or commentary in connection with the image(s).
I authorize the use of these images without compensation to me. All negatives, prints, digital reproductions and shall be the property of Conscious Athlete Academy.
